Semen Analysis Results - Apr 13 2026

SA results are back and the recovery is real. Attaching the full tracker image below.

Latest Numbers (Apr 13)
  • Concentration: 82.8M/mL (was 16.8M in Jan = 4.9x recovery)
  • Volume: 5.2mL (best result to date)
  • Total Motility: 58% (up from 45% in Jan)
  • Progressive Motility: 39% (in range)
  • Morphology: 4% (up from 1% in Jan, at ref floor now)

The Recovery Story

Back in January my concentration crashed to 16.8M, barely scraping above the reference range. Morphology dropped to 1% normal forms. This was the PED damage fully showing up on paper.

Started HCG 500IU 4x/week on Jan 28 alongside HMG. Switched HMG to Puregon 75IU 4x/week on Mar 5 (same FSH stimulation, but I got it prescribed from my doctor so with PBS is much cheaper). Exemestane 12.5mg 2x/week to keep E2 in check and not tank it.

94 days later the concentration is at 82.8M. Full spermatogenesis takes about 74 days so technically the sperm in this sample were already being produced when the protocol was barely a few weeks in. The HCG/Puregon combo kicked the Leydig and Sertoli cells back into gear fast.

Volume is the best its been at 5.2mL which tells me the accessory glands (prostate, seminal vesicles) are responding well to the hormonal environment. Motility tracking up across the board. Morphology still borderline at 4% but up from 1% which was the worst of it.

Not a full natural recovery since I'm still on TRT dose Test E 40mg 2x/week, but the gonadotropin stimulation (HCG mimicking LH + Puregon providing FSH directly) is doing the job even with exogenous test suppressing the HPGA.

Next SA probably in 6-8 weeks to see if concentration keeps climbing and whether morphology can push above 4%. The IVF clinic is happy with these numbers for now.

If you're coming off a cycle and worried about fertility, the bloods don't lie. HCG alone can get you moving but adding FSH stimulation (HMG or Puregon) is what really accelerates the timeline. Don't wait for the full 74 day spermatogenesis window to feel hopeless, the cells start responding much earlier than the textbooks suggest.

Friday 10 April - Update (Upper B from Thu 9 Apr)

Upper B yesterday, volume day with three supersets. Hit all 8 exercises, 26 sets in about an hour. DB incline bench topped out at 40 per side for 8. Everything felt solid.

Protocol: No changes.
- Test E 40mg 2x/week (Mon, Thu)
- HCG 500IU 4x/week (Sun, Mon, Wed, Fri)
- Puregon 75IU 4x/week (Mon, Wed, Fri, Sun)
- Exemestane 12.5mg 2x/week

Fertility: Still waiting on PGTA results. Booking in for a semen analysis on Monday to see where the numbers are sitting now. Last one was 19.2M concentration with 48% motility so hoping to see some more improvement.

E2 management: Feeling good at the moment. No nipple sensitivity, no bloating, brain fog from earlier this week has cleared up. Exemestane at 12.5mg seems to be the right dose. Will confirm with bloods next week.

Sleep was about 9.5 hours (10pm to 7:30am). Body tried to wake me at 6 but I pushed through for another 1.5hrs. Energy 8/10, mood 9/10, libido 8/10. Meals on track.
